Summary

The facility failed to ensure that all Certified Nursing Assistants (CNAs) received at least five hours of dementia-specific in-service training annually, as required. Record reviews and interviews with the Director of Staff Development (DSD) revealed that out of 108 sampled CNAs, attendance at various one-hour dementia training sessions ranged from 33 to 68 CNAs per session, with no evidence that all CNAs completed the required training hours.
