Summary

Surveyors observed multiple failures in the facility's food service operations. The dining room was found to have dirty dishes with leftover food left overnight, and gnats were seen flying around the dishes while residents were present waiting for coffee. Additionally, there was a buildup of grease along the vent hood rails in the kitchen.
