Summary

The facility failed to develop and implement a comprehensive care plan addressing all of a resident's needs, specifically regarding smoking. Upon admission, a resident with diagnoses including hypertension, hyperlipidemia, and a history of alcohol abuse was identified as a smoker who required supervision and the use of a smoke apron during smoke breaks, as documented in his smoke evaluation. However, the resident's care plan did not include any information about his smoking status or required precautions.
