Summary

The facility failed to report an allegation of neglect involving one resident. According to the facility's Protection from Abuse policy, all allegations of abuse or neglect must be reported to the appropriate state agencies, including the Department of Health and Department of Aging. A review of the resident's grievance document showed that the resident reported to the Nursing Home Administrator that a nurse aide was mean to her and refused to provide care on a specific morning. Additionally, the Assistant Director of Nursing/Infection Preventionist documented hearing the resident allege that the nurse aide was rough with her during care. Despite these allegations, there was no evidence that the facility reported the incident to the required state authorities.
