Surveyors observed that the facility failed to maintain cleanliness in the walk-in freezer, as evidenced by the presence of a small clear bowl with a dried orange substance, a moderate to large amount of brown and orange debris on the floor and at the floor-wall edge, and several food items and an unidentified bag under a metal rack.
