Summary

Surveyors observed multiple unsanitary conditions in the facility's kitchen and food service areas during an initial tour. Specific findings included a broken oven kick plate resting on the floor, food debris on the bottom shelves of two preparation tables, and rust with missing protective coating on three storage racks in the dry storage room.
