The Housekeeping Supervisor indicated that staff cleaned and mopped the room with the urine odor daily and used an odor eliminator, but believed the mattress was the source of the smell and needed replacement. The facility did not have a specific policy addressing a homelike environment, though a general resident rights policy was in place.
