Summary

The facility failed to maintain a clean and homelike environment for two residents as evidenced by multiple environmental deficiencies observed during the survey. In one resident's room and bathroom, surveyors noted missing tiles along the wall near the sink and toilet, as well as rust-colored discoloration near the bathtub faucet and brown staining on the wall tiles. Both the Regional Plan Operator and the Environmental Services Manager acknowledged that the condition of the tiles and bathtub did not meet acceptable standards for cleanliness.
