Summary

The facility failed to conduct annual performance evaluations and competency assessments for a Certified Nursing Assistant (CNA) who had been employed since 2013 and worked the night shift. Record review and interviews with the Director of Staff Development (DSD) revealed that there were no records of annual competency training for this CNA for the years 2022, 2023, and 2024.
