Summary

A deficiency was identified when a resident's room was observed to have a sticky, stained, and dirty floor on multiple occasions. The resident, who had a history of major depressive disorder and moderate cognitive impairment, expressed dissatisfaction with the cleanliness of her room, specifically noting that the area beside her bed had not been cleaned despite her complaints.
