Summary

The facility failed to meet the regulatory requirement of providing a minimum of 3.2 hours of direct nursing care per resident per day on five specific days. On February 9, 14, and 15, 2025, and April 20 and 22, 2025, the facility's nursing care hours fell short of the required minimum. Specifically, on February 9, 2025, the facility provided 328.5 care hours for 110 residents, resulting in 2.99 hours per patient day (PPD). On February 14, 2025, 333 care hours were provided for 115 residents, totaling 2.9 PPD. On February 15, 2025, 323.75 care hours were provided for 113 residents, totaling 2.87 PPD. On April 20, 2025, 352.75 care hours were provided for 111 residents, totaling 3.18 PPD. On April 22, 2025, 337 care hours were provided for 109 residents, totaling 3.09 PPD. Interviews with the Nursing Home Administrator and the Director of Nursing confirmed that the staffing levels on these days did not meet the required minimums.

A - N/A B - Staffing Manager or designee will staff the daily nursing staff to meet the required ppd minimum 3.2 for direct res care. C - NHA, DON, and Staffing Coordinator educated on PA required PPD requirement. D - Weekly x 4 then monthly x 2 audits by NHA or designee on PPD levels. Results discussed during QAPI meetings.
