Summary

The facility failed to maintain a portable fire extinguisher in accordance with NFPA 10 standards. During an observation on April 8, 2025, at 8:05 a.m., a portable fire extinguisher was found sitting on the floor next to the Administrator's office. The mount for the extinguisher had been pulled out from the wall and was hanging loosely. This deficiency was confirmed during an interview with the Interim Facility Administrator and the Plant Operations Manager later that day at 1:30 p.m. This issue affected one of the seven smoke compartments in the facility.

Plan Of Correction

1. The unhung fire extinguisher had no negative effects. 2. The fire extinguisher adjacent to the Administrator's office has been rehung on the wall.
