Failure to Provide Required Abuse Training to New Employee

Summary
The facility failed to provide mandatory training on Abuse, Neglect, and Exploitation to a newly hired Physical Therapist, Employee E3, on the date of orientation. According to the facility's policy, such training is required upon hire and annually for all employees. Employee E3 was hired on March 31, 2025, and worked five days in April 2025, but her personnel record did not include documentation of the required training during her orientation. This deficiency was confirmed during an interview with Human Resources Employee E5 on April 17, 2025.
Plan Of Correction
1. No residents experienced adverse effects by failing to provide pre-employment abuse training. E3 will have education. 2. Abuse training will be provided during the pre-employment orientation program. 3. NHA/designee to re-educate current employees on the Abuse, Neglect, and Exploitation Policy. 4. NHA/designee to audit new employee orientation folders 1x/week for 4 weeks, then monthly x 2 months. 5. Results to be submitted to QAPI for review and approval.
