Summary

The facility failed to provide medically related social services to a resident, identified as R128, who was admitted with a diagnosis of depression, hypokalemia, and seizure disorder. The resident was prescribed Selegeline Transdermal Patch for depression, but the medication administration record and clinical progress notes indicated missed doses over several days. Subsequently, the medication was discontinued without prescribing an alternative treatment for depression. Furthermore, there were no referrals made for psychiatric services to address the resident's depression diagnosis. The Director of Nursing confirmed the discontinuation of medication and the lack of referrals during an interview.

Plan Of Correction

1. Resident R128 will be seen by psych services. 2. NHA to educate Social Services/designee on behavioral health services and interventions. 3. Social Services/designee to conduct whole house audit of psych diagnoses to assess for behavioral health services and interventions. 4. SSD/Designee will audit new admissions weekly for 2 weeks, then 1x/week for 2 weeks to ensure residents who are in need of psych services as a baseline receive them. 5. Results to be submitted to QAPI for review and approval.
