The facility failed to provide necessary behavioral health interventions for a resident, identified as Resident 16, to maintain the highest practicable mental well-being. During an interview, Resident 16 expressed feelings of being neglected, stating that no psychology services had been provided since their amputation. The Director of Nursing (DON) acknowledged that the psychology provider typically visits every Tuesday but failed to evaluate Resident 16. The DON mentioned that the psychology provider was hesitant to engage due to fear of the surveyors. Both the Nursing Home Administrator and the DON confirmed the facility's failure to provide the required behavioral health interventions for Resident 16, as evidenced by the lack of psychiatric evaluation and the resident's expressed distress.
Plan Of Correction
1. Resident R16 will be seen by psych services. 2. NHA to educate Social Services/designee on behavioral health services and interventions and ensure psych services list is discussed at IDT meeting. 3. Social Services/designee to audit psych list to ensure services are provided as scheduled. 4. Results to be submitted to QAPI for review and approval.
