The Director of Nursing acknowledged that the care plan should have been updated once the resident's needs changed. The lack of care conferences and failure to update care plans placed residents at risk for unmet care needs and other negative health outcomes. The facility's policy requires care conferences to be held within 48 hours of admission, quarterly, and upon request by the resident or their representative. The absence of a Social Services Director and failure to adhere to policy contributed to these deficiencies, as confirmed by staff interviews.
