Summary

The facility's failure to develop a care plan and ensure communication among staff members about the resident's suicidal ideation and risk factors contributed to this critical event. The Director of Nursing acknowledged that the message from the nurse practitioner regarding the resident's risk was not communicated, and a care plan was not developed to ensure the resident's safety.
