Summary

The facility failed to ensure that equipment was in safe operating condition for a resident, as confirmed by an LPN. Additionally, the facility did not maintain an effective resident call system in one of the three resident restrooms in the East building. Observations on two separate occasions revealed that the Two East Solarium/common area restroom lacked an emergency call light or call cord, which is necessary for emergency use. The facility's policy dated 10/1/24 requires that a call light system be used to respond to residents' requests and needs, and it should be plugged in at all times. However, during interviews, staff confirmed the absence of the required call system in the specified restroom.

Plan Of Correction

1. The call light chord in the 2 east solarium bathroom was replaced and operational. 2. The director of maintenance or designee audited resident bathrooms on 2 east to ensure call light chords were present and operational. 3. The director of maintenance was re-in-serviced by the NHA to ensure call light chords are present and operational and or a proper intervention is in place in the event of a disruption of service. 4. The director of maintenance will audit daily for one week to ensure call light chords are present and operational. Audits will be shared with QAPI committee.
