The facility failed to provide a comprehensive review of resident admission rights and maintain proper admission documentation for Resident R247. The facility's policy on resident rights, last reviewed on October 1, 2024, mandates the protection and promotion of each resident's rights, including informing them about their rights and responsibilities. However, upon review of Resident R247's records, it was found that there was no admission packet or discussion documented that included essential information such as patient portion liability, daily rate cost structure, resident rights, appeal rights, obligations to pay from resident resources, and the processes for Medicare and Medicaid. This lack of documentation and communication was confirmed by the Admission Director, who acknowledged the facility's failure to meet the required standards. Resident R247, who was admitted on October 4, 2022, had diagnoses including dementia, anxiety disorder, and hypertension. Despite these conditions, the facility did not maintain an admission record for the resident, as confirmed by the Business Office Manager. Additionally, the Director of Social Services noted the absence of official Power of Attorney documents for Resident R247. The resident was eventually discharged on August 20, 2024, due to non-payment, highlighting the facility's failure to adequately inform and document the resident's admission rights and financial obligations.
Plan Of Correction
1. The residents responsible party was made aware per dated documentation of the patient liability. The New Admissions Director sent the Resident R247's, who was admitted in October 2022, the admissions packet to the responsible party for resident R247. 2. The new admission Director reviewed the last 30 days worth of admissions to ensure they received a copy of and review of the admission packet with the ability to sign or refuse. 3. The New Admissions Director was re in serviced on communicating and issuing the admission packet to residents and or responsible parties on admission. 4. The New Admission Director will audit daily for two weeks to ensure incoming residents receive a copy of and review of the admission packet with the ability to sign or refuse. Audit findings will be shared with QAPI committee.
