Failure to Meet Minimum Nursing Care Hours
Penalty
Summary
The facility failed to meet the regulatory requirement of providing a minimum of 3.2 hours of direct resident care per resident in a 24-hour period on four specific days within a 21-day timeframe. A review of nursing time schedules from January 16, 2025, to February 5, 2025, revealed that on January 16, 25, 26, and 28, the facility provided less than the required hours, with recorded hours of 3.18, 3.04, 3.14, and 3.11 respectively. This deficiency was confirmed during an interview with the Nursing Home Administrator on February 6, 2025, who acknowledged the shortfall in meeting the mandated nursing care hours on these dates.
Plan Of Correction

We are working with Lenape to create a clinical site which will expand our visibility in the nursing community. We have rented a billboard to advertise our job offerings. Armstrong continues to work with an outside agency. The DON or designee will provide the staffing coordinator with education on the Pennsylvania staffing requirements. Staffing coordinator or designee will audit the PPD daily times 4 weeks, and monthly times 1 month. Audit results will be reviewed through the monthly QAPI process/meeting.
