LPN Staffing Shortages on Day and Night Shifts

Summary
The facility failed to meet the required staffing levels for Licensed Practical Nurses (LPNs) on two separate occasions. On January 19, 2025, during the day shift, the facility did not provide the minimum of one LPN per 25 residents, as only 3.94 full-time equivalents (FTE) were present for a census of 99 residents, when 3.96 FTEs were required. Similarly, on January 13, 2025, during the night shift, the facility did not meet the requirement of one LPN per 40 residents, with only 2.44 FTEs present for a census of 105 residents, when 2.63 FTEs were needed. The Nursing Home Administrator confirmed these staffing shortages and acknowledged that no additional higher-level staff were available to compensate for the deficiency.
Plan Of Correction
The facility cannot retroactively correct past staffing issues. To prevent this from reoccurring, the Scheduler will be reeducated on staffing Licensed Practical Nurses (LPN) to include expectations of LPN ratios by the DON/ designee. The facility will hold staffing meetings 5 days per week, consisting of the Nursing Home Administrator, Director of Nursing, Human Resources, and Scheduler, to review LPN ratio compliance for upcoming schedules. DON/ designee will monitor LPN ratios 5 days a week and ongoing to ensure compliance. Ad hoc education will be provided as needed. The results of the meeting will be forwarded to the facility QAPI committee for further review and recommendations.
