Summary
The facility failed to maintain self-closing devices on doors to hazardous areas, as evidenced by an observation on January 29, 2025. During the inspection, it was noted that a door next to the Carrier room did not self-close when tested. This failure was due to the closer being disconnected from the door frame. The issue was confirmed in an interview with the Facility Administrator and Maintenance Director later that day.
Plan Of Correction
Door closure by the carrier room was fixed on January 30th, 2025. Doors with self-closures will be inspected 1 floor per week for 6 weeks, then monthly to ensure that they are functioning. Results will be reported to the quality assurance and performance improvement committee.
