Summary
The facility failed to ensure that food was served under sanitary conditions, as evidenced by several observations in the kitchen and dishwashing areas. On January 7, 2025, surveyors observed a large amount of water accumulated on the floor underneath two steam kettles in the main kitchen cooking area, which extended to an area where staff could walk. Additionally, in the dishwashing area, there was a significant amount of water on the floor under and around the sides of the dishwasher, with a noticeable leak coming from a pipe on the side of the dishwasher. Condensation was also noted on the ceiling, with water dripping onto the floor. Further observations in the walk-in freezer on the lower level of the dietary department revealed a buildup of ice on the floor in front of the racks against the back wall. An interview with the Dietary Manager confirmed that one steam kettle had a broken seal causing water to leak, the dishwasher had a leak but was still functioning, and there was an accumulation of ice on the floor in the walk-in freezer that should not have been there. These conditions indicate a failure to maintain professional standards for food service safety.

This Plan Of Correction does not constitute an admission for purposes of general liability, professional malpractice or any other court proceeding. 1. No one individual was identified regarding the kitchen equipment leaking water. 2. Current residents and new admissions have the potential to be affected. The kitchen area will be maintained in a sanitary manner. 3. To prevent recurrence, contracted maintenance service to repair water leaks for the steam kettles and dishwasher. Maintenance department removed the accumulated ice from the walk-in freezer floor. 4. To prevent recurrence, Administrator/designee contacted the contracted preventative maintenance service to provide timely preventative/repair services as agreed. 5. To maintain and monitor compliance, Administrator/designee to monitor contracted preventative maintenance/repair service to ensure performance meets expected standards within the contractual language. Administrator/designee to report findings to Quality Assurance Committee.
