Maple Heights Health and Rehab Center was found to be non-compliant with the requirements for the frequency of meals and snacks at bedtime as per 42 CFR Part 483.60(f)(1)-(3). Plan Of Correction

- Preparation and submission of this Plan Of Correction is required by state and federal law. This Plan Of Correction does not constitute an admission for purposes of general liability, professional malpractice or any other court proceeding. 1. No one individual was identified in the untimely delivery of meal tray carts. 2. Current residents and new admissions have the potential to be affected. Residents will receive their meals in accordance to the meal delivery schedule. 3. To prevent recurrence, Dietary manager/designee will in-service the dietary department on the need to deliver meal tray carts within 15 minutes of the scheduled meal times. 4. To maintain and monitor compliance, Dietary manager/designee will audit meal tray cart delivery times 5 times per day, 5 days per week for 4 weeks, 3 times per day, 5 days per week for 2 weeks, 5 times per month for 2 months and 5 times per month for 1 month upon meeting compliance. 5. Dietary manager/designee will report findings to the Quality Assurance Committee monthly until compliance is met.
