Summary

The facility was found to be deficient in displaying written information on how to apply for Medicare and Medicaid benefits, as well as information on receiving refunds for previous payments covered by these programs. This deficiency was observed on two nursing units, specifically the second and third floors. The required postings were located on a bulletin board near the facility's entrance, but residents would need to navigate a staircase of nine steps to access this information, making it difficult for them to view it easily. During an interview, the Nursing Home Administrator confirmed the failure to post the required information in accessible locations on the second and third floors, which is a violation of resident rights as per 28 Pa. Code: S201.29(i).

Plan Of Correction

Postings were placed on the second and third floor on information on how to apply for Medicare and Medicaid. Education on postings will be provided to the social worker and BOM by the NHA or designee on or before 2/11/2025. Audits will be conducted on postings to ensure they are present by DON or designee weekly x 4 weeks and monthly x 1 month. Audit results will be reviewed through the monthly QAPI process/meeting.
