Summary

The facility was found to be deficient in posting required information accessible to residents on two of its nursing floors, specifically the second and third floors. Observations revealed that while the entrance bulletin board displayed the necessary postings, residents would need to navigate a staircase of nine steps to access this information and return to the main floor. The postings included essential details about the State Agency, Adult Protective Services, Medicare Fraud Unit, and instructions on filing a complaint with the State Agency. During an interview, the Nursing Home Administrator confirmed the absence of these postings in accessible areas on the second and third floors, acknowledging the facility's failure to comply with the requirement to make this information easily accessible to residents.

Plan Of Correction

Postings were moved from the front entrance bulletin board to the main hallway. Additional postings were added to the second and third floor to include information on State Agency, Adult Protective Services, Medicare Fraud Unit, and how to file a complaint with state agency. Education on postings will be provided to the social worker by the NHA or designee on or before 2/18/2025. Audits will be conducted on postings to ensure required postings are present by DON or designee weekly x 4 weeks and monthly x 1 month.
