Summary

The facility failed to ensure adequate supervision and proper use of assistive devices, resulting in a fall for a resident during a transfer. The incident occurred when certified nurse aides (CNAs) were transferring the resident from a Broda wheelchair to a bed using a full body lift. During the transfer, the sling became caught on a part of the wheelchair, leading to an imbalance when the CNA attempted to free it.
