The RCP failed to document these changes, including the discontinuation of monitoring for an AV fistula that was no longer in use. The registered nurse acknowledged that the RCP should have been updated to reflect the resident's current status and hospitalization details, but believed the existing interventions were still relevant. The facility's policy on Comprehensive Person-Centered Care Plans requires that care plans be revised as residents' conditions change, particularly after hospital readmissions.
