The facility failed to maintain professional standards in medication administration for two residents. For one resident with a history of atrial fibrillation and on anticoagulant therapy, the facility did not administer Eliquis as ordered due to the medication being out of stock. The Licensed Practical Nurse (LPN) did not notify the physician about the missed dose, which could have led to alternative arrangements for medication. The medication was reordered on a routine basis rather than emergently, and the physician was not informed of the situation until later. Another resident, who was prescribed Lorazepam for anxiety, experienced multiple discrepancies in medication administration records.
