Summary

The facility failed to develop comprehensive care plans for two residents, which led to deficiencies in addressing their specific medical needs. Resident #46, who was cognitively intact and receiving anticoagulant medication, did not have a care plan in place to monitor the effects and potential side effects of the medication. The resident expressed dissatisfaction with not being included in a recent dental visit list, and staff interviews revealed that the social services director was responsible for dental services but had not developed a care plan for these needs. The director of nursing and regional director of clinical services acknowledged the importance of care planning for dental services to ensure staff were aware of the resident's use of dentures and glasses. The facility's policies required comprehensive, person-centered care plans to be developed and implemented for each resident, including measurable objectives and timeframes. These care plans were to be reviewed and updated regularly, especially when there were changes in the resident's condition. However, the facility did not adhere to these policies for the two residents in question, resulting in a failure to meet their highest practicable physical, mental, and psychosocial well-being.
