Summary

The facility failed to accurately document the provision of incentive spirometry (ISP) for three residents, resulting in falsification of medical records. The deficiency was identified through observations, interviews, and record reviews. Residents had medical doctor orders for ISP to be administered every shift, but the treatment was not provided. Despite this, the Medication Administration Records (MAR) indicated that the ISP was given, which was confirmed to be false by multiple facility nurses during interviews. Interviews with several facility nurses revealed that ISP equipment had not been available for at least three months, yet the MARs continued to reflect that the treatment was administered. Nurses admitted to documenting ISP as given or refused, despite not having the equipment to provide the treatment. The DON confirmed that the documentation did not reflect the actual care provided and that the facility's policy and procedure for nursing documentation were not available upon request.
