Summary

The facility failed to implement its written policies and procedures regarding prohibiting and preventing abuse, resulting in a serious incident involving two residents. Resident #1, a female with severe cognitive impairment and a history of dementia, was sexually assaulted by Resident #2, a male resident with no cognitive impairment and a history as a registered sex offender. The incident occurred when Resident #1 was found naked in Resident #2's room, with Resident #2's hand between her legs.
