Interviews with facility staff, including CNAs, RNs, and administrative personnel, revealed that the care plans were not updated correctly due to errors in completing a new wandering evaluation form. This form, when filled out incorrectly, resolved the care plans, removing the documentation of the secure unit placement and elopement risks. Staff acknowledged the importance of having these interventions documented in the care plans to ensure that all staff members were aware of the residents' needs and how to care for them appropriately.
