Summary

The facility failed to implement an effective infection prevention and control program, specifically in the application of Enhanced Barrier Precautions (EBP) for two residents. Resident 5, who had a chronic wound on their left ankle, did not have an EBP sign on their door, and staff were not using gowns and gloves for high-contact care activities.
