Summary
The facility failed to provide abuse prevention training to four agency employees, as determined through staff interviews and a review of employee personnel records. Employees 1, 2, 3, and 4, who were agency staff including a Licensed Practical Nurse, a Registered Nurse, and two nurse aides, reported that they had not received training on the facility's abuse prohibition policy prior to assuming their duties.
