Resident 30, with a history of stroke and chronic kidney disease, was prescribed amoxicillin-Pot clavulanate for a UTI caused by MDR Klebsiella pneumoniae, despite the organism's resistance to amoxicillin. The prescription was not aligned with the culture results, indicating a lack of appropriate antibiotic selection. These deficiencies highlight the facility's failure to ensure residents received effective antibiotic treatments based on culture and sensitivity reports.
Removal Plan
- Residents in the facility would be assessed for UTI symptoms and those assessed to have UTI symptoms would be placed on alert charting and the provider notified for recommendations.
- Review of residents who were treated for a UTI to ensure the residents' UTIs were treated with an appropriate antibiotic based on the Culture and Sensitivity Reports. The provider would be contacted regarding any changes in antibiotic therapy as indicated.
- Residents in the facility on hospice services or on comfort measures would have Physician Orders for Life Sustaining Treatment (POLST) forms reviewed regarding their wishes for treatment, including antibiotics, to ensure the information on the POLST form remained accurate to the residents' current wishes.
- Licensed Nurses would be educated on follow-up required for residents who complain of symptoms consistent with a UTI including provider notification. Daily morning clinical review process would be updated to include a review of any urinalysis tests completed to be followed up daily until the Culture and Sensitivity report was available to ensure antibiotics ordered were appropriate. Providers would be notified of the Culture and Sensitivity results as well as what antibiotics residents were currently administered if applicable.
- Staff education would be completed on reporting resident complaints or potential changes in condition to the charge nurse for follow up.
- Nurse managers would be educated on the need to review a resident's POLST wishes related to antibiotic treatment as indicated for residents on hospice or comfort services if an infection developed.
- The DNS or designee would audit residents treated for UTIs to ensure the Culture and Sensitivity reports were reviewed and followed up on as they became available, and the appropriate follow-up was done if the ordered antibiotic was not effective.
- The consultant pharmacist would review antibiotic use for UTIs and the accompanying Culture and Sensitivity results to ensure appropriate antibiotics were prescribed. Findings would be reported to the QAPI Committee and Medical Director. Reviews would continue ongoing if indicated.

The facility failed to maintain its antibiotic stewardship program and did not document monthly antimicrobial monitoring, tracking, trending, or utilization review. For one resident with epilepsy and parkinsonism, Macrobid was ordered for a UTI, but there was no documented McGeer Criteria assessment or supporting clinical signs and symptoms before the antibiotic was started; the culture later showed mixed normal urogenital flora and the antibiotic was discontinued after lab review.

The infection preventionist reported the family continued to request prophylactic antibiotics even though the resident had no signs or symptoms of UTI, and that education had been provided by nursing staff, the NP, and urology. The pharmacist confirmed the medical record lacked documentation to justify prophylactic antibiotic use and that the order had no end date or reassessment date. The infection preventionist also reported the resident was experiencing increased peripheral edema believed to be a side effect of the antibiotic, but the physician continued the order despite no identified UTI symptoms. The facility’s antibiotic stewardship policy required monitoring of antibiotic use, identification of potentially inappropriate or prophylactic use, and tracking of outcomes, but the report identified that medical staff continued to order antibiotics even when criteria were not met.
Failure to Administer IV Antibiotic and Complete Ordered Weekly Labs

The EHR showed no lab results, and the DON stated the weekly labs had not been completed until the ID office called the facility to report they were missing. The DON also stated she did not know why the IV antibiotic had been discontinued, and the antibiotic was later restarted after the call from the ID office. The resident missed 27 ordered doses of antibiotics.

The SDCF was completed after the antibiotic course was finished, and the progress notes did not document communication with the physician about the urine culture results. Resident 77 had diagnoses including dementia and carrier or suspected carrier of MRSA, with severely impaired cognitive skills and impairment of both upper extremities. The physician ordered urine UA and culture and sensitivity, but the laboratory record showed urinalysis results without urine culture results. The resident was then ordered Macrobid 100 mg twice daily for UTI for seven days and received the medication, while the SDCF showed Criteria 1 was checked and Criteria 2 was left blank. The progress notes did not document communication to the physician about the absence of urine culture, and the SDCF was completed and signed after the antibiotic course had already been completed.

The facility policy on Antibiotic Stewardship - Orders for Antibiotics stated appropriate indications for antibiotics include criteria met for clinical definition of active infection or suspected sepsis, and pathogen susceptibility based on culture and sensitivity, or therapy begun while culture is pending.
Failure to Monitor Antibiotic Use and Document Criteria for Treatment
