Summary

The facility failed to provide adequate supervision and follow individualized care plans, resulting in avoidable accidents for two residents. Resident 1, who had a history of stroke and left-sided hemiparesis, required extensive assistance for bed mobility and incontinence care as per their care plan. However, a CNA assisted Resident 1 alone, contrary to the care plan's requirement for two-person assistance. During this unsupervised care, Resident 1 fell from the bed, sustaining a head injury that required hospitalization.
