The deficiency was identified for a resident who was admitted to hospice care in September 2023, but the most recent hospice notes available in the facility's records were from December 2023. This lack of updated documentation indicated a failure in communication between the facility and hospice care, which is essential for coordinating care and ensuring the resident's needs are met. During interviews, staff members acknowledged the absence of recent hospice notes in the resident's records.
