Similarly, for Resident 10, who was discharged to the hospital due to emesis and abdominal pain, there was no documentation in the EHR showing that the resident or their representative was notified in writing of the reason for discharge. Staff interviews confirmed that the required written notifications were not provided to the resident, their representative, or the Ombudsman. The Director of Nursing acknowledged that the facility had not been notifying residents and their representatives in writing of the reason for discharge, nor sending a notice to the Ombudsman, despite the facility's policy requiring it. The Administrator expressed an expectation for staff to follow the discharge/transfer policy.
