Resident 4, who was admitted to the facility, stated they had an advance directive, but a review of their Electronic Health Record (EHR) showed no documentation of it. Staff C, the Social Services Director, confirmed that if a resident had an advance directive, it should be scanned into the EHR, but no such documentation was found for Resident 4. Staff A, the Administrator, also expected a copy of the advance directive to be in the medical records if it existed. Similarly, Resident 21 stated they had an advance directive and that their Power of Attorney (POA) carried a copy.
