The facility's policy on the proper use of bed rails emphasized the importance of assessing the risk of entrapment and ensuring the correct installation and maintenance of bed rails. However, the failure to document complete assessments for Zones 6 and 7 for the residents in question indicates a lapse in adhering to these guidelines. The Administrator and DON were informed of these deficiencies and acknowledged the findings during interviews.
