The Director of Nursing (DON) confirmed the absence of bed hold notifications for these residents during the survey process. The facility's Bed Hold/Ombudsman Notification Documentation Policy, dated December 2018, mandates that the bed hold policy be provided to the resident at the time of transfer and to the resident's representative within 24 hours. Additionally, the policy requires documentation that the Ombudsman is notified via the monthly transfer log for all hospital transfers and therapeutic leaves. Despite this policy, the facility failed to document and provide the necessary bed hold notifications for the affected residents, as confirmed by the DON during interviews and record reviews.
