Summary

The facility failed to prevent a mentally impaired resident from exiting the facility unsupervised and did not provide adequate supervision and a secure environment. The resident, who had severe cognitive impairment and a history of elopement, was able to leave the facility through a window. The resident's medical record indicated he had multiple diagnoses, including osteomyelitis, type 2 diabetes with foot ulcer, cognitive communication deficit, and schizoaffective disorder.
