Summary

The facility failed to provide adequate supervision and monitoring for a resident who was at risk for elopement. The resident, who had a history of cerebral infarction, mild cognitive impairment, and other medical conditions, was documented as having moderate cognitive impairment and was identified as at risk for elopement. Despite this, the resident managed to leave the facility multiple times on the same day.
