The facility failed to ensure that a missing item was logged and investigated for one of the residents, specifically Resident 23. The resident reported a missing electric toothbrush, and their representative informed the facility about it. Despite this, the grievance was not logged in the facility's grievance log from December 2023 through April 2024. Interviews with staff revealed that the appropriate grievance form was not filled out, and the incident was not properly investigated or documented as per the facility's policy.
