Summary

The facility failed to properly prevent and contain COVID-19, leading to several deficiencies in infection control practices. During an observation, a CNA did not perform hand hygiene between dirty and clean tasks while providing incontinence care to a resident. The CNA used the same gloves to handle both soiled and clean items, and failed to wash hands before donning new gloves. This lapse in protocol was confirmed by the DON, who indicated that hand hygiene should be performed between tasks.
