The facility also failed to provide the April Behavior Monitoring Flowsheet (BMF) and did not document behaviors requiring the necessity for the as-needed Diazepam order. Interviews with staff revealed that the resident exhibited behaviors such as yelling and grabbing chairs, but these were not documented properly in the progress notes or care plan. A late entry note was made after a conversation with staff, but it did not include details about notifying the resident's responsible party or the outcome of non-pharmaceutical interventions.
