Observations showed that Resident 81 struggled to open the door due to its heaviness and the incline leading up to it. He had to use the call light for assistance to exit the bathroom. The Licensed Nurse and the Director of Nursing acknowledged that the door was too heavy for Resident 81 to open independently, posing a risk for falls and feelings of claustrophobia. The facility's policy on Resident Accommodation of Needs was not followed, as residents did not have easy access to the bathroom. The Director of Nursing confirmed that the expectation was for residents to open bathroom doors independently to maintain their level of independence and dignity. The failure to ensure accessible bathroom doors led to feelings of entrapment and the inability to maintain independence with activities of daily living for the affected residents.
